2. Owner: The person or corporation who owns the sign.

3. Address: The street address of the sign.

3b. Additional Site Details: Where the site is on the property, something that is very important for large, multi-sign properties.

4. Condition: Structure Number grade from 1 (poor) to 5 (excellent) Surface same} Lighting same

5. Form: What kind of sign is it? Pylon, fascia, etc.

6. Specfic Description: Detailed description of the sign and its elements.

7. Type of Display: What kind of lighting? Neon, incandescent, LCD, etc.

8. Media: Is the sign steel? Plastic? Brick?

9. Non-neon treatments: What non-neon elements does this sign have?

10. Animation: Does the sign have animation/flashing? Moving parts?

11. Environment: What is the sign near? What is its physical environment?
